Q: How far apart should these be mounted? Exactly 19 inches?

Depending on what equipment you'll be putting in the rack. One schematic I saw said 19 1/64", while for me I put them at 19 5/16", because I have a couple of cisco switches that have about a 1/4" extra because of the rack ears.
